Candidates must have at least 5 years of experience working with individuals with intellectual disabilities and possess an active Medication Administration Program (MAP) certification. A bachelor's degree is preferred, and a valid driver's license with a reliable vehicle is required for travel.

We are seeking a dedicated and experienced Shared Living Homefinder to support our growing Shared Living program (currently serving 10 individuals). Based out of our West Springfield office with travel across Hampden County, you will determine provider eligibility, match individuals with intellectual disabilities and brain injuries with safe, stable host homes, and ensure ongoing regulatory compliance.
