The RN Case Manager is responsible for delivering and evaluating the plan of care for hospice patients through home visits. This includes completing accurate documentation, attending patient meetings, and coordinating care with physicians and the interdisciplinary group.
Candidates must have at least one year of clinical RN experience and a current RN license in the state of service. Additional requirements include CPR certification, a valid driver's license, and access to a dependable vehicle.
