Hinds LIfeCare, one of the most well-known and well-respected nonprofit agencies in California’s Central Valley and a pioneer in the practice of hospice and palliative care, is seeking a Full Time, Medical Social Worker / LCSW to join our team.
Under operational direction of the performs the role of social worker as a member of the interdisciplinary team. The Hospice Social Worker completes psychosocial assessments and provides ongoing psychosocial support to patients and families.

Qualifications

Minimum one (1) year of Medical Social Work experience required; Palliative, Hospice and or Home Health Care preferred
Electronic medical record experience preferred
Ability to learn and assimilate the EHR Computer Program/System
Demonstrated ability to work with health team personnel Bilingual (English/Spanish) a plus but not required
Must have a valid drivers license and a willingness to use personal vehicle in the course of employment
Must show proof of automobile insurance, proof of current auto registration and acceptable driving record (no more than 3 points within a 36 month period)
Ability to meet the physical demands of the job (ie, Push, Pull, Lift/Carry 10 lbs)

Hinds Hospice, founded by Nancy Hinds, R.N., provides expert end-of-life hospice medical care in Fresno, Madera, and Merced counties, since 1981. OUR PROGRAMS Hinds Hospice operates the only freestanding six-bed hospice home in Fresno, Madera, and Merced Counties. When patients' symptoms become too complicated to be cared for in their home, the Hinds Hospice home in Fresno offers around-the-clock nursing care. The Hinds Hospice Center for Grief and Healing is unique in that it serves not only hospice families, but also the community at large with grief support services. We offer one-on-one grief counseling, group support specific to the type of loss, monthly grief mailings, or someone to listen on the phone. The news of an unborn baby being diagnosed with a life-limiting condition is heartbreaking. Angel Babies offers a perinatal hospice program designed for families who have received this diagnosis, offering support and comfort during pregnancy and providing ideas for creating loving memories. Children require different care than adults, so our innovative pediatric hospice program allows for concurrent treatment along with hospice services.
