The Visit Nurse coordinates and oversees interdisciplinary plans of care for patients in home care and hospice settings. This involves working collaboratively with a multidisciplinary team to provide care for an average of 4-6 patients per shift within an assigned territory.
Candidates must have an Associate's Degree in Nursing, a valid Wisconsin or eNLC compact RN license, and a valid driver's license. BLS certification is required, while a Bachelor's degree, home health experience, and EPIC software proficiency are preferred.
The Home Health & Hospice Visit Nurse works collaboratively with patients, nurses, social workers, physicians, other practitioners, caregivers and community agencies in coordinating and overseeing the interdisciplinary plan of care for a patient in a home care and home hospice environments. In this role, you can expect to work within an assigned territory providing care for an average of 4-6 patients per shift.
Ideal candidate possesses excellent interpersonal communication and negotiation skills along with strong analytical and computer skills. Prior home health is beneficial.
