They also maintain clinical records, serve as a liaison with community agencies, and participate in the development of the Plan of Care.
Candidates must have a Master's Degree in Social Work from an accredited institution and an active MSW Professional License from Georgia. A minimum of one year of experience in healthcare, preferably in hospice or hospital settings, is required.

At Longleaf Hospice and Palliative Care, we integrate and address the diverse, seen and unseen needs of patients and their families.
A member of the hospice interdisciplinary team, the Licensed Master Social Worker assists the team in recognizing and understanding the social/mental stress and/or disorder that magnifies the symptoms related to the terminal illness.
