The RN Acute Care Navigator oversees patient care progression and coordinates the appropriate level of care to optimize length of stay and prevent readmissions. They collaborate with interdisciplinary teams to conduct assessments, develop discharge plans, and provide essential patient education.
Candidates must hold a current Registered Nurse license and either a BSN or five years of applicable nursing experience. An MSN and specialty certifications in care management (such as CCM or ACM) are preferred.
A ProMedica RN Acute Care Navigator plays a key role in overseeing patient care, seamless care progression, coordination, and the appropriate level of care for their caseload. You focus on optimizing length of stay, preventing readmissions and compliance with value-based programs.
You will collaborate with interdisciplinary teams to conduct initial assessments and develop discharge plans.
This role also requires patient education through providing patients with information on medications, treatments, and post-discharge care. Through the patient's stay, you will monitor their progress, review daily health records, and escalate any issues to leadership to ensure smooth transitions and patient outcomes.
