The Hospice Social Worker provides psychosocial services, comprehensive assessments, and bereavement counseling to terminally ill patients and their families. They collaborate with an interdisciplinary team to develop care plans and facilitate referrals to community resources.
Candidates must hold a Master's degree in Social Work from a CSWE-accredited school and possess an active Ohio Social Work license. A minimum of one year of healthcare experience working with terminally ill patients is required.
