The GIP RN manages hospice admissions and supports inpatient palliative care services within contracted hospitals. They act as a liaison between hospital staff, patients, and families to ensure seamless transitions to home care or other facilities.
Candidates must hold an active nursing license from an accredited program and possess strong knowledge of hospice care. A valid driver's license and automobile insurance are required, with healthcare marketing experience preferred.

Location: Sunrise Hospital
Schedule: Monday - Friday 10:00 am - 7:00 pm
The GIP RN for Infinity Hospice Care (IHC), works primarily in hospitals where IHC holds a current contract for hospital GIP and supports the hospital inpatient palliative care service. This GIP RN gains a high understanding of the IHC intake process and support transitions to both home IHC and NEVPC.
The GIP RN promotes the hospice program, enrolls patients, and supports providers when needed to transition patients to IHC and NEVPC. The GIP RN engages patients, families, caregivers, and updates hospital staff as needed. The GIP RN acts as a liaison in supporting documentation, notes, and electronic information transmission to both IHC and NEVPC.
This position reports directly to the Director of Patient Care Services (DPS) of IHC as well as any assigned hospital contracted Providers and Advance Practice Nurses.
