Job Summary
Work on a multidisciplinary team providing behavioral health and crisis assessment to consumers at The Providence Center and in the community.
Duties & Responsibilities
Perform a comprehensive psychosocial assessment of assigned consumers. -Treat consumers with respect. -Perform in a calm and professional manner during crisis or emergency situations. -Apply knowledge of Psychiatric Diagnosis (DSM V TR). -Perform and record mental status examination. -Assess clinical emergencies, including suicide and danger to others. -Collect pertinent data from appropriate collateral sources in accordance with confidentiality guidelines. -Maintain confidentiality in accordance with Center policy and legal requirements. -Develop and present, verbally and in writing, a clear clinical formulation based on behavioral data and relevant theory while incorporating psychosocial and family issues. -Use psychiatric consultation and intervention when clinically appropriate. -Use knowledge to decide appropriate levels of care in order to refer to various points in the continuum of care.
Conduct level of care and crisis assessments to clients and non-clients in a variety of settings including homes, shelters, street outreach, etc. Response to active police / fire scenes required with safety protocols.
Collaborate with other providers, agencies, and individuals in the consumers' network of care. -Maintain knowledge and familiarity of mental health and/or substance abuse and other related community agencies. -Communicate with various local agencies and individuals, as well as with Center staff.
Perform record keeping in accordance with Health Information Services and other Center requirements. -Complete crisis assessments. -Complete initial assessment and integrated summary forms. -Complete progress notes with appropriate format. -Develop and implement a plan of care with ongoing consumer input. -Complete a clear, specific, behaviorally oriented treatment plan as required with consumer signature. -Complete insurance documentation as required. -Complete required reports and evaluations. -Follow required office procedures in a cooperative manner.
Attend trainings, case presentations and conferences. -Provide feedback to staff on the content of educational programs attended. -Participate in the education of other Center staff. -Attend mandatory in-service trainings and other trainings required for renewal of licensure.
Maintain cooperative relationships with Center staff, consumers, community agencies and the public. -Communicate with various local agencies and other departments or teams at The Center. -Work in cooperation with co-workers and supervisory staff. -Deal with team members regarding problems in a constructive manner. -Benefit from constructive criticism.
OCCASIONAL DUTIES: Serve on appropriate Center committees.
Provide mental health consultation to other community agencies.
