Licensed Practical Nurse (LPN)
Under the direction of the RN, the Licensed Practical Nurse (LPN) delivers high-quality, patient-centered nursing care within a collaborative Primary Care Team. The LPN supports health promotion, disease management, and care coordination while advancing Fair Haven Community Health Care’s mission of providing accessible, culturally competent healthcare.

With a commitment to serving the New Haven, Branford and East Haven communities, FHCHC takes a holistic approach to health, linking medical services, pharmacy services, dental services, and behavioral health. FHCHC has grown from the small store front to a complex, multifaceted primary care center that currently serves more than 36,000 patients with over 150,000 visits per year.
