Support individuals experiencing homelessness to build independence and reach their full potential. This includes assessing needs, updating support plans, and collaborating with external partners to improve health, wellbeing, and employment prospects.
Candidates should have experience supporting adults experiencing homelessness and possess strong organisational and IT skills. An NVQ Level 3 in Health and Social Care is required or a willingness to work towards achieving it.
