The Registered Nurse coordinates individualized care for patients and families within the community, performing comprehensive physical and psychosocial assessments. They work within an interdisciplinary team to implement care plans, facilitate transitions, and provide ongoing education and emotional support.
Candidates must hold a current Registered Nurse license and a BLS certification, with at least two years of nursing experience. A bachelor's degree is preferred, along with experience in community-based settings like hospice or home health.
