The Assistant Case Manager provides psychosocial assessments, develops treatment plans, and coordinates referrals for veterans. Additionally, they manage transportation, facilitate community programming, and maintain facility safety and sobriety standards.

Requirements

Candidates must have a high school diploma or equivalent and a valid driver's license. Experience in customer service and the ability to pass a drug screen and background check are required.

The Mission of Veterans Place is to empower veterans' transition from homeless to home, ending the cycle of homelessness, and to assist all at-risk veterans to become engaged, valuable citizens who contribute to their communities. Our vision is a community where veteran homelessness is rare, brief, and non-reoccurring and where veterans are valued, productive, and respected citizens who contribute to their community. Veterans Place is a CARF Accredited organization, following the ASPIRE model, to provide quality, value, and optimal outcomes of services to our clients.

Position Responsibilities:

Time Commitment and Duty:

Veterans Place of Washington Boulevard is staffed 24 hours a day 7 days a week. The Assistant Case Manager (ACM) must be available during all days and shifts and to work holidays. Compensation is made for holidays worked. The typical schedule will be focused on these hours when other staff members are not on the property.

 

Transportation:

  • Acts as the primary van driver when on duty and provides transportation services to veterans according to program, appointment, and community needs.

 

Community Development and Programming:

  • Assesses needs and interests of Veterans Place residents by interacting with them on a regular basis.
  • Initiates programs, activities and community development that address the social, recreational, educational and development aspects of the residents' lives and promote positive relationships, sobriety and supportive community engagement.
  • Assists with orientation and welcoming of new residents.
  • Works with staff members to promote responsible living and a culture of sobriety at Veterans Place and is a positive role model in both professional and personal behavior.
  • Assists in collection and delivery of drug screen samples to the Veterans Administration for processing.
  • Monitors physical facilities, conducts building rounds as needed and performs light maintenance and grounds tasks (e.g., switching blown breakers, spreading ice melter on walks, etc.).

Communication:

  • Communicates important and arising issues to Milieu Coordinator and, where needed, participates in development of an appropriate course of action.
  • Communicate policies and the rationale behind them to residents.
  • Participate in administrative, staff, clinical, and in-service activities and training as required.

 

Clinical Activates:

  • Perform case finding functions and outreach, identifying and screening patients for psychosocial needs.
  • Conduct psychosocial assessments and develop treatment plans in collaboration with the veteran and other provider organizations including the Veterans Administration.
  • Coordinate referrals of Veterans to the Veterans Administration and other community-based providers for additional services based on needs and eligibility (this includes expanding the existing referral network as necessary).
  • Exercise and coordinate a variety of individual and group counseling techniques that are evidence based and demonstrate good clinical practice.
  • Maintain case records in accordance with program guidelines and professional requirements.
  • Implement legal and ethical standards of service delivery, privacy, and confidentiality, complying with all applicable regulations.

Qualifications:

  • High School Diploma or equivalent
  • Experience working with the public and/or in a customer service capacity strongly preferred.
  • Ability to relate well to others and handle administrative responsibilities.
  • Must have a valid driver's license
  • Veteran Status preferred
  • Passing a pre-employment drug and alcohol screening is required and a background check will be conducted

Founded in 1996 and located in Larimer, we strive to provide a variety of supportive services for the growing population of homeless veterans in the Pittsburgh area. The goal of Veterans Place, working in partnership with the VA Pittsburgh Healthcare System and other organizations, is to facilitate Veterans’ transition from homeless to formerly homeless, becoming engaged, valuable citizens who contribute to their communities. In our current location since 2004, we operate several programs in this pursuit. First, we offer homeless Veterans a place to live here in our 48-bed Transitional Housing Program. This program provides up to a 24-month stay in one of our 13 townhouses on Washington Boulevard. Veterans in this program receive case management, recovery support, clothing, an on-site food pantry, employment support, and other services designed to build them toward self-sufficiency and successful independent living. Second, we serve Veterans living in Pittsburgh’s emergency shelters and on the streets through our Homeless Veterans Day Program. Each day, Monday through Friday, our shuttle picks up Veterans at homeless shelters throughout Pittsburgh and transports them here to our Service Center. Once here, we provide them with breakfast, lunch, clothing, case management, referrals, and services designed to address the root issues that cause homelessness, including chemical dependency and mental illness. We are also able to provide other transportation support, including shuttle service for medical appointments and access to other community resources. At Veterans Place, our goal is to provide homeless Veterans with an array of facilities and services that build long-term residential stability, employment, sobriety, and self-determination. Through our Veterans Place services and those delivered through a variety of community partnerships, we strive to be a full-service agency for all homeless Veterans.
