THORNHILL COMMUNITY HEALTH CENTRE

The Thornhill Community Health Centre (CHC) is an interdisciplinary primary care and community health organization committed to providing accessible, inclusive, and proactive care for individuals and communities facing barriers to care.
Thornhill CHC incorporates accessibility-focused, disability-informed, and equity-centred approaches to care, with a particular focus on supporting individuals with intellectual and developmental disabilities (IDD), neurodiverse individuals, seniors, caregivers, and other underserved populations.
The CHC works collaboratively with community partners, healthcare providers, and interdisciplinary teams to improve access to comprehensive, person-centred care and promote health equity across the community.

About the position
The Nurse Practitioner (NP) is a key member of our interdisciplinary primary care team, delivering comprehensive, client-centered healthcare services to individuals and families across the lifespan.
Working to their full scope of practice, the NP provides assessment, diagnosis, treatment, prescribing, health education, care planning, and ongoing management of acute and chronic health conditions. The role involves collaborating closely with physicians, nurses, social workers, psychotherapists, care navigators, and community partners to ensure coordinated and integrated care.
As part of a growing Community Health Centre, the NP will contribute to the development of clinical programs, care pathways, and quality improvement initiatives while helping to shape innovative and accessible healthcare services for the community.

Hours of Work
This position follows a CHC team-based primary care model with scheduled clinical hours as follows:
  • Monday–Wednesday: 9:00 a.m. – 5:00 p.m.
  • Thursday: 8:00 a.m. – 8:00 p.m.
  • Friday: 9:00 a.m. – 1:00 p.m.
  • Sunday: 9:00 a.m. – 1:00 p.m. (approximately once per month)
Total weekly hours: 37.5 hours (CHC standard). A 30-minute unpaid lunch break is scheduled Monday to Friday.
During the initial operational phase, these hours will apply as outlined above. Once the clinic is fully operational, hours may be adjusted in alignment with service needs and interdisciplinary team-based scheduling.
This role also includes periodic participation in shared after-hours on-call coverage with physicians and nurse practitioners (typically a few days every few months).

Key Responsibilities

Comprehensive Primary Care
  • Provide comprehensive primary care services within the Nurse Practitioner scope of practice.
  • Conduct comprehensive health assessments.
  • Diagnose and manage acute and chronic illnesses.
  • Order, perform, and interpret diagnostic tests.
  • Prescribe medications and therapeutic interventions.
  • Monitor treatment effectiveness and adjust care plans as required.
  • Provide episodic, preventative, and ongoing primary care services.
  • Deliver evidence-informed care based on best practice guidelines.
  • Provide assessment, treatment, education, and ongoing management for clients with chronic and complex conditions, including individualized care planning, self-management support, and interdisciplinary collaboration to improve health outcomes.
  • Provide assessment and support for clients experiencing mental and behavioural health concerns, including anxiety, depression, trauma, grief and loss, adjustment disorders, behavioural challenges, and substance use issues.
  • Participate in crisis intervention and risk assessment as required.
  • Collaborate with the Social Worker / Registered Psychotherapist and other team members to provide integrated mental health care.
  • Provide care to priority populations facing barriers to health and wellbeing, including seniors, caregivers, individuals with disabilities, newcomers, people experiencing housing or financial insecurity, and equity-deserving communities.
Clinical Leadership and Program Development

As part of a new CHC, the NP will:
  • Assist in developing and refining clinical workflows and procedures.
  • Support implementation of new programs and services.
  • Participate in quality improvement initiatives.
  • Contribute to policy and procedure development.
  • Support accreditation activities.
  • Participate in strategic planning and organizational initiatives.
  • Identify opportunities to improve client access, experience, and outcomes.
Documentation, Reporting and Quality Improvement
  • Maintain accurate, timely, and professional clinical documentation within the Electronic Medical Record (EMR).
  • Complete encounter documentation and CHC coding requirements (e.g., ENCODE-FM).
  • Support reporting requirements for Ontario Health, Alliance for Healthier Communities, and other funders.
  • Participate in program evaluation and quality improvement activities.


Skills, Knowledge and Expertise

1) Qualifications
  • Registered Nurse in the Extended Class (RN-EC) with the College of Nurses of Ontario.
  • Completion of a recognized Nurse Practitioner educational program.
  • Current registration in good standing with the College of Nurses of Ontario.
2) Experience Required
  • Minimum of 2–3 years of clinical experience as a Registered Nurse and/or Nurse Practitioner, preferably within primary care, community health, family practice, public health, hospital, or related healthcare settings.
  • Experience providing care to clients with chronic and complex health conditions.
  • Experience working with vulnerable and marginalized populations.
  • Experience supporting individuals with mental health, behavioural, and psychosocial challenges.
  • Experience working within interdisciplinary teams.
  • Experience using Electronic Medical Records (EMRs).
