The Case Manager provides housing-focused support to shelter guests by orienting them to the facility and helping them identify goals. They are responsible for coordinating services, maintaining regular contact with guests, and ensuring accurate documentation.
The position requires strong communication, organizational, and professional skills to work effectively with individuals facing homelessness and various barriers. Candidates must be able to manage housing plans and connect guests with necessary community resources.
The Case Manager provides housing-focused case management and individualized support to guests of the Porchlight Men's Shelter. This position enrolls and orients new shelter guests, and helps guests identify goals, address barriers, connect with community resources, and pursue permanent housing.
Case Managers are responsible for maintaining regular contact with assigned guests, developing and updating housing plans, coordinating services, and completing accurate and timely documentation. The position requires strong communication, organization, professionalism, and the ability to work effectively with individuals experiencing homelessness, mental illness, substance use disorders, medical concerns, and other barriers to housing stability.
Shift Hours Saturday through Wednesday, 1pm-9pm

We are the largest non-profit provider of affordable housing in Dane County and are comprised of an emergency shelter for men, an eviction prevention program, an outreach team, a job-training program, and provide housing and supportive services to men, women, and families.
