Assist individuals with disabilities in accessing social, vocational, and volunteer opportunities to promote independence and community integration. Responsibilities include implementing Individual Service Plans and documenting personal health information in a case management database.
Requires a Community Support Worker Diploma or equivalent and at least 2 years of experience in community social services or healthcare. Candidates must possess a valid driver's license, First Aid/CPR and Food Safe certifications, and be physically capable of supporting clients.

The services are determined by the person and based on their specific goals and preferences. You’ll encourage and empower the person to be independent and self-determining and to live the life they envision while maintaining excellent standards of care, health, and safety. Your natural ability to care for others and work collaboratively towards a goal will allow you to influence your person’s success.
