We pride ourselves in community integration with the support we provide through various programs, which include, eight residential settings, day programs, habilitation, and group supported employment. New Horizons is looking for a Direct Support Professional (DSP) to empower individuals with lifelong disabilities to enjoy life by promoting abilities and respecting personal choice.
Direct Support Professional (DSP) Responsibilities:
Assist individuals with activities of daily living, including personal hygiene, dressing, grooming, toileting, bathing, and mobility as needed.
Prepare nutritious meals, assist with meal planning, grocery shopping, and light housekeeping to maintain a clean, safe home environment.
Administer medications and complete medication documentation in accordance with organization policies and training.
Transport and accompany individuals to medical appointments, community activities, shopping, and recreational outings.
Encourage participation in community events, hobbies, employment, volunteer opportunities, and other meaningful activities based on individual goals.
Complete daily documentation, progress notes, medication records, and incident reports accurately and on time.
Observe and report changes in an individual's health, behavior, or overall well-being to the appropriate supervisor.
Follow individualized support plans, behavior support plans, and all safety procedures.
Build positive, respectful relationships while promoting independence, self-advocacy, and person-centered supports.
Communicate professionally with coworkers, supervisors, families, healthcare providers, and other members of the interdisciplinary team.
Maintain all required certifications and attend mandatory trainings.
Qualifications
High school diploma or GED required.
21+ years of age
Valid driver's license with an acceptable driving record.
Ability to pass all required background checks.
Ability to lift, transfer, and assist individuals as needed.
Previous experience supporting individuals with intellectual and developmental disabilities or caregiving experience is preferred but not required.
