About the Role

You will be responsible for providing culturally sensitive health education and case management/care coordination services to pregnant and newly parenting women, including assessment, referrals, follow-up monitoring, and needed support. Services are provided during regular business hours, in the evenings, and on Saturdays as needed.

Core Functional Responsibilities

  • Provides intensive case management, including assessment, referral, follow-up, and monitoring to moderate and high-risk pregnant women, new parents, and their families, through routine in-person home, virtual, and office visits and regular phone contact.
  • Supports clients in planning for their own health and social service needs by assisting clients in identifying goals.
  • Refers participants needing more intensive follow-up to appropriate agencies.
  • Learns about and makes contact with community resources needed by clients.
  • Conducts health promotion activities by providing client education, disseminating educational literature, and distributing disease prevention materials, including condoms.
  • Ensures timely scheduling of prenatal, postpartum, well-child, and early intervention appointments.
  • Assists in organizing client meetings and other special events. 
  • Attends community meetings, health fairs, and represents MCC as requested by supervisor.
  • Keeps careful records of all contacts made and follows up as necessary. 
  • Keeps accurate records of services received by clients. 
  • Routinely uses MCC client database and other computer systems, including e-mail, Internet, and word processing.
  • Attends orientation and ongoing training sessions on maternal and child health and welfare, child abuse, housing, nutrition, substance abuse, counseling, parenting, and infant and family support issues.
  • Meets with supervisor regularly to report on and discuss work.
  • Ensures confidentiality of client records, sharing pertinent information only with the client’s approval. 
  • Conducts outreach activities to support client recruitment, as assigned.

Supporting Responsibilities

  • Adheres to the MCC’s security guidelines and ensures the appropriate handling of sensitive information.
  • Attends relevant staff meetings to promote communication and execution of goals.
  • Completes special projects specific to the function of the department or as needed for the department, as directed by the supervisor.
  • Other duties as assigned within the scope of position expectations.

 

Knowledge, Skills & Abilities

  • Understanding MCC’s mission, goals, and objectives, and ability to work independently with a high level of energy and contribute as part of a larger team.
  • Knowledge of the concepts and practices associated with community health issues, particularly with pregnant women, prenatal care, parenting, and other services for mothers, young children, and their families.
  • Ability to travel in and around the Greater Philadelphia and Montgomery County regions to conduct home visits and community outreach.
  • Knowledge of and sensitivity to issues facing the community served.
  • Experience in client management procedures for determining eligibility, assessing needs, identifying resources, making referrals, following up, and documenting client interactions.
  • Strong interpersonal skills as demonstrated by compassionate, courteous, cordial, cooperative, and professional interaction with diverse groups of co-workers, community partners, clients, and members of the community.
  • Ability to operate a computer and use a variety of common software programs, including Microsoft Office, spreadsheets, and customized databases.
  • Effective and accurate documentation of case notes and client data according to established client management documentation procedures.
  • Ability to organize tasks efficiently and follow up and follow through with strong attention to detail.

Maternity Care Coalition operates as a hybrid organization. The assigned work arrangement for this position may include a combination of remote and onsite work and will be determined based on operational needs, role responsibilities, and performance. Work arrangement expectations may be modified over time to meet organizational or programmatic requirements. Staff working remotely are responsible for maintaining a safe and appropriate workspace.

  • Ability to engage in extended periods of sitting, computer work, and sustained concentration to complete financial analysis, reporting, and related tasks.
  • Ability to occasionally bend, reach, lift, carry, push, or pull training materials or equipment, generally not exceeding 30 pounds.
  • Possession of a valid driver’s license issued by the state of residence and the ability to successfully pass a motor vehicle record check, where driving is an essential function of the role.
  • Ability to travel occasionally to attend conferences, seminars, and professional meetings.
  • Ability to work non‑traditional hours on an occasional basis based on operational needs.
  • Employment is contingent upon the successful completion of required post‑offer pre‑employment background clearances, as well as any additional post‑hire background, compliance, or programmatic clearances required by the organization, funders, or applicable regulations.

Experience, Education, and Licensure

 

Experience

  • Experience providing direct/case management services in a community outreach setting is required.
  • Experience with community health issues
  • Experience working with maternal and child health issues preferred


Education

  • Associate degree in Social Work, Public, or related field; 
  • Or a combination of education and relevant professional experience will be considered in lieu of a degree.
