Provide psychosocial assessments, crisis intervention, and care planning for end-of-life patients and their families in their residences. Coordinate hospice admissions and connect families with available community and agency resources.
Requires an active Maryland LMSW license, a valid driver's license, and reliable transportation. Candidates should have at least one year of experience in hospice or geriatric care.
