The Home Care Worker provides direct personal care and assistance to frail elders in their homes to help them maintain independence and safety. Responsibilities include assisting with daily living activities, monitoring health status, and providing emotional support as part of an interdisciplinary healthcare team.
Candidates must have at least one year of experience working with older adults in either a paid or unpaid capacity. A Certified Nursing Assistant (CNA) certification is desired, and the ability to lift 35 lbs is a physical requirement.
