Conduct thorough physical and history assessments for patients and their families to determine home care needs. Develop and update individualized care plans and manage discharge planning and counseling.
Requires a valid state RN license, BLS/CPR certification, and at least one year of recent acute care experience. An Associate's or Bachelor's degree in Nursing is required, with home health experience preferred.
