The State Care Coordinator provides person-centered care coordination for individuals transitioning from substance use treatment to the next level of care. Responsibilities include conducting client intakes, managing service plans, performing field visits to treatment sites, and maintaining accurate documentation in the client database.
Candidates must have a high school diploma or GED with significant human services experience, while a bachelor's degree is preferred. At least 2 years of administrative support experience, preferably with the substance abuse recovery population, and reliable transportation are required.
