The QIDP will partner with individuals and their families to create and guide individualized service plans while coordinating care. This role involves leading Community Support Team meetings and balancing office work with field-based responsibilities in homes and the community.
Candidates must hold a bachelor's degree in Social Work, Psychology, Special Education, or a related field and have at least one year of experience supporting individuals with disabilities. Eligibility for the DHS/DDD QIDP Registry and a valid driver's license are also required.

The QIDP role is a great position to gain case management experience for recent graduates!
What You’ll Do:
As a QIDP, you’ll partner directly with the people we serve, their families, and support teams to create and guide individualized service plans. You’ll lead Community Support Team (CST) meetings, coordinate care, and ensure services align with each person’s goals and preferences. This role blends office work with hands-on time in homes and the community—making a real impact every day.
