The Social Worker provides psycho-social assessments, crisis intervention, and counseling for residents and their families in a long-term care facility. They also manage the admissions process and coordinate care plan conferences and resident council meetings.
Candidates must hold a Baccalaureate degree in Social Work and be registered with the BC College of Social Workers. One year of recent experience working with the elderly in residential settings is required.
Reporting administratively to the Manager, Clinical Services and clinically to the Clinical Practice Leader, Social Worker, the Social Worker, Long Term Care uses professional and program standards in providing psycho-social assessment, crisis intervention counselling and specific resident treatment, as well as assisting with tasks that are a cross-responsibility of all team members.
Education and Experience
Baccalaureate degree from an approved school of Social Work. One year's recent related experience working with the elderly in residential settings.
Current full registration with the BC College of Social Workers.
