Overview
In accordance withthe College of Nurses of Ontario (CNO) Standards of Practice and as an integral member of the Public Health Services team,providesimmunization services within the framework of the Population Health & Health Promotion Model. Applies knowledge,skillsand abilities specific to Vaccine Preventable Disease, Epidemiology, Community Development and Primary Health Care, Healthy Equity and Public Health Ethics. Translates knowledge from health and social sciences to support immunizations through universal and targeted interventions,programsandadvocacy.
Plans, organizes,implementsand evaluates immunization services forParaMed employees and homebound ParaMed clients. Works in collaboration with the local Public Health Unit, conducts immunization clinics for eligible staffand clients in a variety of settings by assessing needs,establishinginformed consent,preparingand administering injections andobservingand responding to reactions. Following theParaMed Immunization Policyand Medical Directives, the Immunization & Public Health Promotion RegisteredPracticalNurseadvisesclients of adverse reactionsand contraindications and provides after care instructions and related documentation.
Submits Adverse Events Following Immunization (AEFI) reporting form to the local Public Health Unit.Completes immunization recordsanddocumentsimmunization administrationsutilizingPublic Health Unitonline portal and localWW DistrictTracker.Records statistical immunization data andparticipatesin continuous quality improvement initiatives.
Responsibilities
Immunizes staff and adults against vaccine preventable disease
Conducts immunization clinics by assessing immunization needs, transporting vaccines, setting upwork stations,establishinginformed consent,preparingand administering injections andobservingand responding to reactions such as fainting and anaphylaxis to ensure clientsafety.Ensurescold chain procedures are followedin accordance withpolicies and procedures.
Maintains related records;documentsobservations,interventionsand outcomes. Collects statistics, prepares reports asrequiredandin accordance withestablished standards and procedures.
As part of an interdisciplinary/inter-sectoral team,establisheseffective working relationships and partnerships with other professions, community members, and agencies.
Participates in continuous quality improvement initiatives by providing input into the review and revision of immunization policies and procedures and attending meetings to promote improvements to client care and the efficient and safe operation of the program.
Performs other related duties as assigned.
