EveryMind is hiring a BILINGUAL Family Care Manager to join our Linkages to Learning team. This role is responsible for direct care management services within a school and community-based program designed to integrate a system of resources and services for youth and families and to support them in achieving their goals.

Work Location: Weller Road Elementary School; Silver Spring, MD
Schedule: Monday-Friday, mostly on-site during core busines hours
Bilingual (Spanish) Required.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Completes comprehensive needs and strength assessments with youth and families resulting in individualized service plans and goals and appropriate crisis intervention or support as needed.
  2. Consults and collaborates effectively with colleagues, school staff, and other community agencies to provide comprehensive services for clients. Offers information and referral support for community members, school staff, etc.
  3. Provides ongoing social services to clients which may include securing needed resources, providing necessary referrals, and ensuring any court-ordered requirements are met. Assists with completing applications for programs and services that align with service plan goals.
  4. Maintains compliance with School Based Services contract including: Meeting productivity requirements, maintaining minimum caseload requirements, and timely submission of data to support program reporting requirements.
  5. Conducts psycho-educational groups, educational workshops, and resource presentations as appropriate.
  6. Incorporates evidence-based practices and individualizes activities to support students and families with a variety of physical, emotional, and social needs.
  7. Maintains accurate, thorough, and timely documentation of client records. Documents and evaluates services provided and client progress, updates service plan at regular intervals.
  8. Manages funds in assigned budget categories and utilizes assigned client needs funds to support service plan and client goal achievement.
  9. Assists with seasonal events, such as camp registration, school supply drive, holiday giving program, and other activities for the school community.
  10. Provides translation and interpretation services for clients and others as time allows.
  11. Utilizes regular supervision with Manager to evaluate and improve the quality of care management services and ensures that services are culturally appropriate for the client.
  12. Maintains client confidentiality according to EveryMind Confidentiality Policy.
  13. Other duties as assigned.

Occasional supervision of interns or volunteers.

Reports to Manager, Linkages to Learning.

Strong Personal Resilience and Mental Health

Given the high-stress nature of the role, it is essential that candidates possess a high level of personal resilience and maintain stable mental health. This ensures that they are well-equipped to provide effective support and maintain the professional standards required for the position.

Education

Bachelor’s degree in social service field required.

Experience

1 year of successful experience in the human services field, required.
Experience with case management service delivery preferred.

Skills

Strong interpersonal and communication skills. Demonstrated organizational and community building skills. Ability to
manage multiple priorities. Accurate record keeping and data entry proficiency. Bilingual proficiency in Spanish/English required.

Abilities

Effectively interact and work collaboratively in a flexible work environment with a variety of constituents including CEO,
board members, clients and family members or caretakers, interns, volunteers, staff, and community partners. Ability to
express ideas with clarity, confidence, and sensitivity. Must be able to maintain confidentiality involving clients in
accordance with organizational and federal guidelines.

Physical and Emotional Demands

The physical demands described here represent those that an employee must meet to successfully perform the

perform the essential functions. While performing this job's duties, the employee is frequently required to stand, walk,
sit, use hands to finger dexterity, handle with hands and arms, climb stairs, balance, kneel, crouch, or talk and hear.
The employee must be able to lift and/or move up to 35 pounds frequently. The job's specific vision abilities include
close vision, distance vision, and the ability to adjust focus.

Due to the nature of the content that is handled in the program, anyone who has a history of trauma or suicidality could
be at risk of being triggered or re-traumatized. This is especially true for people who have experienced these issues
within the past 12 months and therefore we may not recommend this work if their experience is so recent.

Clearance Requirements

The employee must pass a criminal background check. Certain job functions may require employee to pass a child
sexual abuse and other criminal activity as well as a credit check. All staff providing services to staff or clients, must be
free of illegal drugs and abuse of alcohol, and must be fully able to always perform all required duties.

Working Conditions

The work environment is representative of that which an employee encounters while performing the essential functions

functions.

The employee must be available to perform the duties of the job during most, but not limited to core business hours
(between 9-5pm, M-F), some flexibility with schedule may be available (evening and/or weekend hours). Employees will
work mostly on-site but may work some hours remotely with the approval of the supervisor.

The employee is also required to travel to different locations. Therefore, the employee must possess a valid driver’s license and have access to a vehicle.
