The LPN provides behavioral health care to patients in their homes according to the established Plan of Care and physician orders. Responsibilities include monitoring patient conditions, documenting medication regimens, and collaborating with RN Case Managers to prevent avoidable hospitalizations.
Candidates must be a graduate of an accredited nursing program with a current State License as an LPN/LVN and a high school diploma or GED. At least one year of clinical care experience is required, with a valid driver's license and the ability to lift 50-100 lbs.
