The RN Admissions nurse assesses patients in their homes or hospitals to initiate care and serves as an ambassador for the organization. They are responsible for completing clinical admissions, obtaining physician orders, and facilitating a seamless transition to care navigation.
Candidates must hold a current RN license and have at least one year of nursing experience, with hospice or hospital experience preferred. Strong clinical assessment skills and proficiency with EMR/EHR systems are required, along with a valid driver's license and active BLS certification.
