The Reconnections Outreach Worker will engage with individuals who are rough sleeping or insecurely housed to facilitate their return to their area of origin. This includes conducting assessments, coordinating resettlement plans, and liaising with various agencies to support vulnerable populations.
Candidates must have a strong understanding of the needs of people experiencing homelessness and knowledge of service pathways for substance misuse, mental health, or housing. Experience or knowledge regarding immigration law, safeguarding, and supporting victims of modern slavery is essential.

The Brighton and Hove Street Outreach Service is recruiting one full time Reconnections Outreach Worker to join the Reconnections Team of the Service. You will be required to work some unsocial hours.
The post holder will form part of a busy outreach team that supports the reconnection of service users to their area of origin. Good communication skills, team-working and an understanding of the issues faced by people experiencing homelessness or vulnerable individuals is essential. Having good knowledge of the city and good partnership links is desirable.
Where: Brighton
Full Time Hours: 37.5 per week
Contract: Fixed term until 31st March 2027
Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
