The Registered Nurse provides care and services to hospice patients in their residences, including private homes and assisted living facilities. They collaborate with an interdisciplinary team to perform psych-social assessments and manage patient care in case management, admissions, or resource roles.
Candidates must hold a current Registered Nurse (RN) license in good standing within the state of practice. Requirements include a valid driver's license, auto insurance, and the ability to work independently while collaborating with a healthcare team.

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Case Management, Admissions, or Resource role.
