Conduct comprehensive aged care assessments to identify the needs of older adults and facilitate referrals to essential support services. Actively promote the retention of functionality in daily living while ensuring all assessments comply with Department of Health contractual requirements.
Requires at least one year of experience working with older Australians in assessments or service delivery. Candidates must hold a tertiary qualification in a health-related discipline, professional registration, and a current driver's license.
As a part-time (4 days per week) Clinical Aged Care Assessor, you'll play a crucial role in identifying the needs of older adults and connecting them with services that help them live independently with the highest quality of life.
Daily travel is a core requirement of this role. You will be required to travel throughout your service area every day to meet business and client needs, providing support where it’s needed most
