The RN provides comprehensive physical and psychosocial assessments to develop individualized care plans for seriously ill patients and their families. They coordinate care with interdisciplinary teams and facilitate transitions between hospital and community settings.
Candidates must have a valid Registered Nurse license, BLS certification, and at least 2 years of RN experience. A bachelor's degree and experience in community-based settings like hospice or home health are preferred.
