Perform comprehensive diagnostic assessments and provide scheduled patient visits for adults in long term care facilities. Maintain up-to-date treatment plans and collaborate with case management and psychiatric service providers.
Requires a Master's degree in social work or professional counseling and a valid LCSW or LMHC license. A valid driver's license, reliable transportation, and preferably 2 years of therapy experience are required.
