Coordinate and manage care for patients with chronic kidney disease, including providing dialysis and transplant education. Handle data collection and reporting for national programs like HALT-CKD and participate in quality improvement audits.
Requires a degree in Nursing or a healthcare-related field with preferably 2 years of relevant experience. Strong communication skills and experience in peritoneal dialysis are highly advantageous.
You will coordinate and manage the care of patients with chronic kidney disease. This includes providing dialysis education and basic transplant education as well as ensuring the smooth transition of patients from the low clearance clinic to dialysis. You will be involved in data collection and submission of reports to the Ministry of Health in support of national programmes such as Holistic Approach in Lowering and Tracking Chronic Kidney Disease (HALT-CKD). In addition, you will participate in quality improvement projects and service quality audits.
