Coordinate patient placement for ED admits, direct admits, and hospital transfers across all Legacy facilities in a 24/7 setting. Monitor bed availability, unit staffing capacity, and facilitate physician-to-physician handoffs to ensure continuity of care.
Requires a high school diploma and at least one year of healthcare experience in admitting, medical records, or clerical support, plus six months of customer service. Preferred qualifications include college-level health records coursework and CHAA certification.

As a key coordinator of patient placement, you ensure that each individual receives timely and appropriate care within our facilities. Your efforts help streamline ED admissions, direct admissions, and hospital-to-hospital transfers—making these transitions as smooth as possible. In everything you do, you embody Legacy’s mission: to make life better for others.
Coordinate patient placement for all incoming patients across all Legacy facilities in a 24/7 operational setting
Collaborate with hospital charge nurses and Nursing Supervisors to ensure accurate placement of:
Emergency Department (ED) admits
Direct admits
Hospital-to-hospital transfers
Manage all aspects of patient transfers from outside facilities into the Legacy system
Facilitate physician-to-physician handoffs to maintain continuity of care during transfers
Act as a liaison for internal and external physicians requesting consults, admission support, or patient information
Monitor and track:
Bed assignments and availability
Unit staffing capacity
Patient discharges
Ensure accuracy of the nightly hospital census report
