The Clinical Transition Educator provides clinical training to patients and caregivers to ensure a successful transition to home infusion care. They also partner with the sales team to increase patient transition volume and coordinate discharge planning with facility staff.
Candidates must hold an active and unrestricted Registered Nurse (RN) license and have at least two years of healthcare industry experience. The role requires reliable transportation for 100% local travel and proficiency in standard office software.

Supervisory Responsibilities

100% local travel to and from partnerships facilities, community hospitals, and medical practice offices to sell Option Care Services, process referrals and provide live education and training support to patient/caregiver and referral sources.
