The MAP coordinates medical care by scheduling appointments, providing transportation, and advocating for the healthcare rights of persons with developmental disabilities. They are also responsible for maintaining accurate medical records and communicating treatment plans to interdisciplinary teams.
A high school diploma and a valid driver's license are required, with at least one year of experience in healthcare coordination or patient advocacy. Certifications in CPR, First Aid, AMAP, and SCIP are preferred.

The MAP is responsible for the coordination of medical care, which includes ensuring timely medical appointments, promoting health advocacy, and enhancing the overall well-being of the persons we support.
