The Case Associate conducts home visits to assess family needs and accompanies clients to community appointments for support and advocacy. They also handle administrative tasks, including documentation and data entry within the agency's electronic health record system.
A Bachelor's degree in social work or a related field is required, with prior experience in child welfare settings preferred. Candidates should possess strong interpersonal skills and proficiency in Spanish is a plus.

To assist in achieving this mission, NAC is currently seeking a Case Associate to join its Special Medical Prevention Program.
Our Prevention Program delivers a range of clinical, case management, and supportive services to families who are child-welfare involved and caring for children diagnosed with chronic or complex medical conditions. The goal is for families to remain intact while ensuring caregivers receive the appropriate support to meet their children's physical, developmental, and mental health needs.
The Case Associate will assist Case Planners with casework activities and administrative case-related tasks. The ideal candidate will be strength-based, trauma-informed, culturally responsive and committed to family preservation and resilience. This is a citywide home and community-based program, and the candidate must have the ability to work independently and flex their hours on weekdays to meet the needs of youth and caregivers. Our hybrid model supports work-from-home when staff are not in the field. This position offers a highly rewarding opportunity to build relationships with underserved families and an exciting opportunity to be part of an innovative multidisciplinary team comprised of pediatricians, nurses, psychiatrists, psychologists, clinicians, case associates, education and resource specialists, and more!
