The Care Coordinator (CC) is responsible for coordinating and implementing the wraparound process for children, youth, and families with complex behavioral health, emotional, developmental, and multi-system needs. This position provides intensive care coordination using a family-driven, strength-based, culturally responsive, and trauma-informed approach to service delivery. Additionally, the Care Coordinator partners with youth, families, multidisciplinary teams, and community agencies to develop individualized plans of care that promote stability, resiliency, safety, and successful functioning in home, school, and community settings. The position ensures coordination across behavioral health, medical, educational, juvenile justice, child welfare, and social service systems while advocating for family voice, youth engagement, and access to appropriate supports and resources.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES

Facilitate the wraparound process for assigned children, youth, and families, including assessment, planning, implementation, monitoring, transition, and closure planning.
Conduct comprehensive psycho-social and strength-based assessments to identify needs, risks, supports, and service gaps.
Develop, implement, and monitor individualized, person-centered, culturally responsive, and cost-effective plans of care.
Utilize trauma-informed care, motivational interviewing, and family engagement strategies to support positive outcomes and strengthen family participation.
Engage families and youth as equal partners, ensuring family voice and choice remain central to all planning and decision-making processes.
Maintain ongoing collaboration and coordinated communication with multidisciplinary providers, systems, and community partners involved in the youth and family’s care, including schools, care management organizations (CMOs), courts, juvenile justice, child welfare agencies, mental health clinicians, physicians, parent-peer support specialist and other community-based providers, while ensuring stakeholders receive timely updates and appropriate notice of Child and Family Team meetings and wraparound planning activities.
Monitor client progress toward identified goals, evaluate service effectiveness, and revise care plans as clinically indicated.
Develop, implement, and present 24-hour crisis and safety plans during quarterly case conferences and as needed.
Provide crisis intervention, crisis de-escalation support, and linkage to emergency or community-based resources when necessary.
Assist families in accessing traditional and non-traditional services, natural supports, and community resources.
Advocate for youth and families while helping them navigate behavioral health, educational, legal, and social service systems.
Attend required court hearings and provide written and verbal updates regarding client progress, recommendations, and service needs.
Facilitate continuity of care and coordinate transitions between services, including recommendations for case transfer or closure when appropriate.
Maintain accurate, timely, and compliant documentation including assessments, service plans, progress notes, crisis plans, court reports, and discharge summaries in accordance with agency, Medicaid, state, and funding requirements.
Provide coaching, training, job shadowing, and educational support to new hires regarding the wraparound process and care coordination practices.
Participate in supervision, administrative meetings, committees, training courses, and quality improvement initiatives.
Maintain compliance with MiSide-Agency Policies, Professional Ethics, confidentiality standards, and all applicable local, state, and federal regulations.

MINIMUM JOB REQUIREMENTS

Bachelor's degree in social work, Psychology, Counseling, Mental Health, Child Welfare, Nursing, or a related human services field required.
Current State of Michigan limited or full professional licensure required.
Candidates whose degree program does not qualify for limited or full professional licensure must obtain and maintain Social Service Technician (SST) registration through the Michigan Department of Licensing and Regulatory Affairs (LARA) upon hire and remain in compliance with all applicable state requirements.
Valid driver's license and reliable transportation required for day-to-day job performance. This position may require transporting clients and families to appointments, services, or other treatment-related activities in accordance with agency policies and program needs.
Minimum of three (3) years of experience working with children, adolescents, and families in behavioral health, child welfare, intensive care coordination, case management, or a related human services setting.
Experience facilitating Wraparound services and working with youth with serious emotional disturbances (SED), behavioral health challenges, or multi-system involvement preferred.
Experience with trauma-informed care, crisis intervention and de-escalation, motivational interviewing, and family systems approaches preferred.
Knowledge of community resources and Medicaid-funded behavioral health services.
Bilingual and/or bicultural experience preferred.

Strong knowledge of the Intensive Care Coordination with Wraparound (ICCW) philosophy of care, trauma-informed practices, and child and adolescent development.

Compensation

Salary is commensurate with education and experience. This is a full-time, union position.
