The MDS Coordinator monitors patient care quality and develops comprehensive care plans in accordance with physician orders. They coordinate services across multiple disciplines and educate patients and families on preventive health.
Candidates must have a nursing degree and 2-4 years of experience as an MDS Coordinator. An active California RN license and BLS/CPR certifications are required.

Responsibilities :
Monitor and assess the quality of care provided to patients.
Develop and implement plans of care for patients in accordance with physician orders and facility policies.
Assist with the development of long-term care plans for geriatric patients.
Coordinate patient care services between multiple disciplines.
Educate patients and families on the importance of preventive health measures.
Provide support to physicians, nurses, and other healthcare professionals in providing patient care.
Maintain accurate patient records.
QUALIFICATIONS
EDUCATION: Nursing degree from accredited college
EXPERIENCE: 2-4 years of MDS Coordinator Experience is REQUIRED
LICENSES/CERTIFICATIONS:
Active RN license in state of California is REQUIRED
BLS and CPR Certification REQUIRED
