The Targeted Case Manager provides care coordination and resource linkage for clients with severe mental illness or substance use concerns. Responsibilities include conducting assessments, developing comprehensive plans of care, and collaborating with community providers to ensure quality health outcomes.
A minimum of a Bachelor's degree in Psychology, Social Work, or Human Services is required along with 1-2 years of experience in behavioral health or social services. Candidates must possess a valid NYS driver's license and access to reliable transportation.

Targeted Case Manager Position Summary:
The Targeted Case Manager (TCM)) is responsible for providing care coordination and resource linkage for clients served by Liberty Resources’ Integrated Healthcare clinic. The TCM plays a key role in managing referrals for new clients and conducting outreach activities to assess and connect individuals with the services they need within the agency. This role primarily supports clients living with severe mental illness and/or substance use concerns, ensuring they receive comprehensive and tailored care to improve their overall well-being.
