Licensed Practical Nurse (LPN) Position Summary:
The Licensed Practical Nurse (LPN)LPN will function as a member the Integrated Health Care Clinical Team in assisting with a full range of nursing/medical services for clients. Under the direction of the registered professional nurse, the LPN will perform tasks and responsibilities within the framework of health teaching, and provision of supportive and restorative care.
Performs on site visits and consultations with clients and staff to determine current health status and any intervention needed.
Provides education and training to clients and staff as indicated by need on health related topics.
Provides health monitoring and screening to clients, including phlebotomy procedures for which LPN is trained.
Provides medical crisis intervention as needed in consultation with Community Mental Health Nurse, Medical Director, or Medical Director’s designee.
Ensures adequate provision and documentation of health services provided at the clinic site.
Assists in providing medication refills, prior authorizations, and tasks related to ensuring prescribed medication regimens are made available to clients per Agency and program policies.
Assists in acquiring updated information regarding clients’ medical health information from approved healthcare providers
Consults with families asappropriate to ensure the communication of pertinent health-related information.
Collects information for reviews as directed by Community Mental Health Nurse.
Assists in maintainingaccurate communication and transfer of health information with health provider and other service providers.
Maintains and updates required records and documents for services provided.
Monitors/instructs staff/clientsregardingpreventative health care and other health related issues as needed.
Handles emergency situations which may require flexibility of time and areas of assignment.
Performs other duties, responsibilities and related tasks as assigned.
Licensed Practical Nurse (LPN) Qualifications:
Graduate of an accredited LPN program with current licensure in NYS.One year of clinical experience as an LPN, with experience working with mental health population preferred. Must have eligibility for full and unconditional participation in the Medicaid and Medicare programs. Excellent verbal and written communication skills. Ability to effectively use an electronic medical record for all documentation.Must have a valid New York State driver’s license and access to reliable transportation when required.
