The Discharge Planner completes initial discharge plans and coordinates transitions of care from the hospital to community resources. They collaborate with Case Managers, Social Workers, and families to ensure timely and cost-effective patient movement.
Requires a high school diploma or equivalent, with a bachelor's degree in social work and BLS certification preferred. Candidates should have strong communication skills and ideally one year of experience in an acute healthcare setting.
