Description

The Interim Housing Weekend Case Manager is responsible for assigned new intakes and assessments, service planning, routine case management services, advocacy and counseling. Case managers meet daily basic needs and provide supportive services as needed to address the tailored needs of homeless individuals and families. The primary focus of the Weekend Case Manager is to provide a stabilizing program presence and to support clients in development of individual client service plans as well as active housing plans. On the weekends, the Case Manager may also complete tasks with clients that the assigned case manager was unable to complete and is responsible for the triage of urgent or unforeseen situations that arise and require stabilization. The Case Manager is also responsible for at least one weekend activity each month. The Case Manager should document client service transactions in HMIS and Salesforce as required. Case Manager should perform operational tasks as necessary and administrative tasks as assigned to fulfill program objectives. 

 

The position is part-time, Saturday and Sunday 8:00am to 4:00pm and 4 hours during the week.  

 

Requirements

Responsibilities 

  • Admitting 
  • Intake and assessment of clients admitted during the weekends. 
  • Perform program intake, including the completion of all necessary Housing Forward forms to ensure proper registration for all new clients. 
  • Provide clients with an orientation to the Interim Housing Program services and rules. 
  • Review with and have resident sign Client Agreement Form. 

Case Management Responsibilities 

  • Provide case management services including advocacy, crisis intervention, and counseling to assigned program participants. The weekend case manager will have a limited caseload of no more than seven clients.  
  • Follow up with residents on tasks that they were unable to complete with assigned case managers. 
  • In conjunction with client, establish housing service plan: review case progress and complete re- determination in accordance with program requirements. 
  • Make referrals in accordance with service plans to internal programs and external providers. Initiate follow-up of referrals to coordinate services and monitor client progress. 
  • Ensure that clients have completed all necessary documentation to be placed on housing lists. 
  • Be aware of potential volatile situations and have a plan to intervene quickly and as early as possible to prevent or mitigate a crisis.  

Administrative 

  • Complete reports of unusual incidents as necessary and submit according to the agency’s protocol.   
  • Read communication logs and other documents to stay informed of the milieu of the program so as to have a solid sense of potential hot issues.  
  • Maintain accurate and confidential records (e.g. case notes) in accordance with agency policies. Responsible for accurate and timely data entry of service transactions into appropriate databases. HMIS (Homeless Management Information System) and Salesforce  

 Other Duties 

  • Attend and participate in clinical case staffing and staff meetings. 
  • Other duties as assigned. 

QUALIFICATIONS: 

Bachelor’s degree in Social Work, Master’s preferred, or related field or minimum of two years relevant experience.  

 

KEY COMPETENCIES: 

  • Knowledge of and experience with de-escalation techniques to manage crisis situations. 
  • Skills or experience in the following areas: Homeless issues, mental health and substance abuse recognition, and accessing supportive services. Consistent, reliable, and team oriented. 
  • Excellent written and oral skills necessary.  
  • Must be able to manage multiple tasks 
  • Solid knowledge of computer programs including Microsoft Word and Excel 
  •  Must have smartphone to utilize Agency communication and collaboration tools. 

PHYSICAL REQUIREMENTS:

The employee must occasionally lift and/or move up to 30 pounds. While performing the duties of this job, the employee is regularly required to use hands and talk or hear. The employee is frequently required to stand; walk; sit; climb stairs, reach with hands and arms balance, stoop, kneel or crouch and work on a computer.
