Provide telephonic triage and support to hospice patients and families during after-hours and weekends. Coordinate with the on-call team and conduct physical patient visits as needed for admissions and routine care.
Must possess a current State Registered Nurse License and a valid Driver's License with auto insurance. Two years of RN experience in a healthcare or clinical setting is preferred.
