The Social Worker will assess hospice patients to identify psychosocial, grief, and community needs while providing counseling and crisis intervention. They are responsible for facilitating community resources, managing advance directives, and ensuring compliance with clinical documentation and regulatory standards.
Candidates must hold a Bachelor's or Master's degree in social work and possess a current state license. At least one year of clinical social work experience is required, along with a valid driver's license and reliable transportation.
