The acute social worker conducts psychosocial assessments and develops comprehensive discharge plans while collaborating with an interdisciplinary team. They also provide emotional support to patients and families and address social determinants of health to facilitate effective care coordination.
Candidates must hold a Bachelor's degree in Social Work and possess a valid Minnesota Social Work license. Less than one year of experience in a healthcare or related setting is required.
Fairview is looking to add a BSW Social worker to our team at the University of Minnesota Medical Center in Minneapolis, MN. The position is requires two weekend shifts a month and one holiday per year.
The acute social worker works on-site with the interdisciplinary teams including providers, physical therapists, nurses, dieticians, chaplains and pharmacists to assist patients and families in navigating through their hospitalization. The acute social worker completes psychosocial assessments, care planning/ progression and creates comprehensive discharge plans from the hospital. The social worker is patient facing, responds to crisis situations, manages patient and family expectations, creates solutions to identified discharge barriers and aims to reduce length of stay and prevent avoidable hospital readmissions.
