Ongoing/Permanent, Full-time or Part-Time Opportunities Available.
Appointment to this position will be at either AHP1 or AHP2 classification, commensurate with the applicant’s level of professional experience, demonstrated competence, and capacity to perform with increasing independence, clinical judgement, and complexity of practice as outlined in the role requirements.
WHAT ARE WE OFFERING?
As an Occupational Therapist, you will support older people across the Barossa Hills Fleurieu Local Health Network as part of a Community Reablement team. The role is varied, with work across clinic, home and residential settings, focusing on maintaining independence, building confidence, and supporting clients to achieve meaningful goals.
You will work with individuals experiencing frailty, dementia, falls risk and chronic health conditions, applying practical, evidence-based interventions to improve everyday function. The position contributes across hospital avoidance, healthy ageing and dementia care programs, offering a broad and engaging scope of practice.
With a solid level of clinical autonomy, you will also support team development, contribute to supervision, and participate in collaborative care planning to ensure consistent, high-quality service delivery.

HOW WILL YOU CONTRIBUTE?
You will..........
Deliver Occupational Therapy services to older people under Commonwealth aged care programs across home, clinic and community settings, with a focus on goal-directed, reablement-based care that supports independence.
Undertake assessment, intervention and care coordination for a complex caseload, including discharge planning, referrals and ongoing rehabilitation, working collaboratively within a multidisciplinary team.
Contribute to team capability through supervision of allied health assistants and students, and participation in service planning, education and professional networks.
Maintain accurate and timely documentation in line with clinical, organisational and funding requirements, including data collection and use of relevant systems.
Participate in service development and continuous improvement activities, including quality initiatives, program development and evaluation, to strengthen outcomes and service delivery.
