The Registered Nurse will conduct scheduled home visits to assess patient needs and execute individualized care plans. They are responsible for maintaining professional patient records and building trust-based relationships to help patients remain comfortable at home.
Candidates must be a Registered Nurse in good standing with the CNO, possess a valid driver's license, and have access to a vehicle. Willingness to travel and provide on-call coverage is required, with bilingualism in English and French preferred.

What you’ll do:
Using appropriate assessment tools/techniques to assess patients’ individualized needs
Executing and continuously monitor care plans
Engaging patients using a collaborative approach by building rapport and trust
Maintaining and update professional patient records related to patient encounters and plans
What you bring:
Registration with the CNO as a Registered Nurse in good standing
Valid driver’s license and access to a vehicle is required
Experience in delivering community care is an asset
Reliability and flexibility
Willingness to travel to and from patient residences within a designated area
Passion for making a difference in your community
Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
