The Registered Practical Nurse will conduct scheduled community visits to provide safe and ethical nursing care to patients of all ages. Responsibilities include assessing individual needs, executing care plans, and maintaining professional patient records.
Candidates must be registered with the CNO as a Registered Practical Nurse in good standing and possess a valid driver's license with access to a vehicle. Experience in community care and working with vulnerable populations is considered an asset.

In this community nursing role, you as a Registered Practical Nurse (RPN) will have scheduled visits within the community to care for patients in various settings with the support of RNs for more complex patients. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible. Execute and continuously monitor care plans and adjust as required

Maintain and update professional patient records related to patient encounters and plans
